Impact of Nurse-Led, Multidisciplinary Home-Based Intervention on Event-Free Survival Across the Spectrum of Chronic Heart Disease

چکیده
There is an increasing pressure to develop cost-effective strategies to mitigate persistently high levels of rehospitalization and premature mortality associated with chronic heart disease. Beyond pharmacological agents and devices, disease management programs are integral to applying flexible, guideline-based management, coordinating care, and providing individualized support. However, the evidence to support their application across the full spectrum of heart disease remains incomplete and fragmented. For example, although the overall evidence supporting short-term cardiac rehabilitation and chronic heart failure (CHF) management programs is extensive and mostly favorable, there is a paucity of research to support programs designed to prevent progressive cardiac dysfunction in patients who survive an acute coronary syndrome or to optimize outcomes in those with atrial fibrillation (AF). Moreover, a diverse range of disease management techniques have been tested. Ideally, the same form of disease management program adapted to the individual patient profile would be tested across a diverse range of cardiac patients receiving high levels of standard care to determine its overall potential to improve health outcomes in the real-world setting.

Extending the horizon in chronic heart failure: effects of multidisciplinary, home-based intervention relative to usual care.
BACKGROUND The long-term impact of chronic heart failure management programs over the typical life span of affected individuals is unknown. METHODS AND RESULTS The effects of a nurse-led, multidisciplinary, home-based intervention (HBI) in a typically elderly cohort of patients with chronic heart failure initially randomized to either HBI (n=149) or usual postdischarge care (UC) (n=148) after...

BACKGROUND It is not known to what extent initially observed benefits of postdischarge programs of care for patients with chronic congestive heart failure (CHF) in respect to event-free survival, readmissions, and healthcare costs persist in the long term. Methods and Results- We prospectively studied the long-term effects of a multidisciplinary home-based intervention (HBI) in a cohort of CHF ...
متن کاملSurvival analysis of breast cancer patients with different chronic diseases through parametric and semi-parametric approaches
Introduction: There is a lack of information on the extent of dependency between chronic diseases and the survival rate of breast cancer. Until date, none of the models proposed has determined the impact of chronic diseases on breast cancer survival. This study, therefore, aimed to investigate the impacts of chronic diseases such as diabetes, blood pressure, and endocrine di...
متن کاملA multidisciplinary telehealth program in patients with combined chronic obstructive pulmonary disease and chronic heart failure: study protocol for a randomized controlled trial
BACKGROUND Chronic obstructive pulmonary disease (COPD) and chronic heart failure (CHF) frequently coexist, significantly reducing patients' quality of life and increasing morbidity and mortality. For either single disease, a multidisciplinary disease-management approach supported by telecommunication technologies offers the best outcome in terms of prolonged survival and reduced hospital readm...
متن کاملEffect of Home-based Pulmonary Rehabilitation on Daily Activity of Patients with Chronic Obstructive Pulmonary Disease
Background: Pulmonary rehabilitation is an important part of treatment in patients with COPD (Chronic Obstructive Pulmonary Disease) with the goal of achieving and maintaining maximum functional independence since this disease causes symptoms including fatigue, and consequently decrease in daily living activity. Aim: To determine the effect of home-based pulmonary rehabilitation on daily living...
